NDIS Support Coordination to Achieve Your Goals

Support coordination is a capacity building support that helps NDIS participants use their plans to achieve their goals.

We provide three levels of support coordination: support connection (level 1), coordination of supports (level 2) and specialist support coordination (level 3).

What is Support Coordination?

How can our support coordinators assist you?

Deepen your understanding of your NDIS plan.

Empower your choice and control.

Connect you to trusted service providers in your area.

Identify and explain all of your support options.

Organise assessments and appointments.

Support you through difficult times and help you apply for more funding if you require extra support.

Assist in monitoring your NDIS plan budget and funding expenditure.

Ensure you maximise the funds available in your plan.

Resolve any problems in relation to your plan.

Facilitate meetings between you and the NDIS if needed.

Founded on the principle that ‘it takes a village’, we partner cooperatively with trusted agencies and individuals to deliver on our clients’ goals.

How do I choose a support coordinator?

There has been a large growth in the disability services sector since the start of the NDIS in 2016, so it can be difficult to know which service provider to choose.

When choosing a support coordinator, you must consider the following:

  • Are they impartial?

  • Do they communicate clearly?

  • How much experience do they have?

  • How well do they understand the NDIS?

  • Do they have a good rapport with you?

  • Are they focused on helping you achieve your goals?

A good support coordinator will have intimate knowledge of the NDIS and be focused on achieving the best support outcomes for you. They will empower you to make decisions over the services you receive, so you can have access to the care you require.
